Adoption Contract
  1. I, the adopter, Adopters Name, agree to take possession of the dog, The Dog's Name, and to treat it with love and compassion as a member of my family as of the date of this contract, and understand that I am entering into a legal and binding contract and violation of any of it's terms are actionable by BRAS and all court costs incurred will be paid for by me the adopter.
  2. I agree to feed the dog at least twice a day, provide fresh water at all times, never leave the dog outside un-attended, crate, kennel or otherwise confine the dog to a small space.
  3. I will provide annual veterinary care for the dog and provide vaccinations as required by my veterinarian and the law of my city, state, and country.  I will provide veterinary care as needed for the safety, health, and well being of my dog and understand that all fees incurred are my sole responsibility upon taking possession of the dog and release BRAS and it's agents from any and all financial responsibility.
  4. I affirm that no member of my household has ever been convicted of an animal welfare law violation such as cruelty, abandonment, neglect, etc.
  5. I am adopting this dog for myself as a companion animal and I agree to not sell, trade, gift, or abandon to a shelter, as long as the dog is alive.
  6. I will notify BRAS immediately if I am unable to care for the dog for any reason and the dog will be returned to BRAS for placement in another home.
  7. I agree to keep identification on the dog at all times and to microchip the dog.
  8. I agree to notify BRAS via email or in writing if the dog should become lost, be stolen, die, or the address of the dog's residence changes.
  9. I agree to permit BRAS to inquire as to the health and well being of the dog any time after adoption, including home visits and if the terms of the contract have been violated, to surrender the dog to a BRAS representative upon request.
  10. I agree to notify a Bras representative of any behavioral problems and to work with the representative to help overcome them before returning the dog, I understand the dog is not house trained or leash trained.
  11. If the dog is returned to BRAS within 2 weeks of the date of this contract, the donation, less a 25% processing fee will be returned to the adopter.
